Opportunities for Improved Clinical Trial Designs in Acute Respiratory Distress Syndrome

Background

Acute respiratory distress syndrome (ARDS) is a common cause of respiratory failure with high mortality rates. The disease can arise from a variety of causes and manifests heterogeneously.

Study Findings

A recent observational study of 459 ICUs in 50 countries showed clinical recognition rates of ARDS ranging from 0.5 to 8.9 per 100,000 population. The study also found that there is significant variation in the clinical presentation and management of ARDS.

Implications for Clinical Trials

The findings of this study highlight the need for improved clinical trial designs in ARDS. To date, most clinical trials in ARDS have been small and have focused on single interventions. However, the heterogeneity of ARDS suggests that a more targeted approach is needed.

Future Directions

Future clinical trials in ARDS should focus on identifying and validating biomarkers that can be used to identify patients who are most likely to benefit from specific interventions. Trials should also be designed to test combination therapies and to evaluate the effects of interventions on long-term outcomes.
